Return to Sport and Activity After Steroid Injection

Corticosteroid injections are a common treatment option for musculoskeletal pain. They may be used to address issues involving joints, tendons, ligaments, and, less commonly, muscles. While we’ve previously discussed when steroid injections may be appropriate and when they should probably be avoided, my general practice is to steer individuals away from their use for most conditions.

However, should you opt for a corticosteroid injection as part of your management plan, it is essential to couple this approach with a deliberate and structured plan to return to function. The primary advantage of steroid injections lies in their ability to provide a significant amount of pain relief. Unfortunately, this relief can create a false sense of security, prompting individuals to return to sports or exercise too quickly.
This premature return can often lead to a worsening of symptoms and the potential for further structural degeneration and, in some cases, even tendon rupture. To mitigate these risks, we generally recommend that individuals wait at least one week before engaging in activities that put considerable pressure on the injected area. Furthermore, it is advisable to wait two weeks before resuming high-intensity exercise.
It’s important to acknowledge that in professional or elite sports, corticosteroid injections may indeed be utilized to reduce symptoms and facilitate an earlier return to play. However, in these circumstances, the risk-reward ratio is notably different. Athletes, along with all parties involved, are fully aware of the associated risks.
